Two duties run in parallel. First, recording: keep a register of injuries, first aid, and near misses, which your health and safety committee reviews and an auditor may ask to see. Second, reporting: a critical injury or fatality generally must be reported to your provincial OHS regulator (WorkSafeBC, Alberta OHS, Ontario MLITSD, CNESST and so on) immediately, and injuries that require medical aid or cause lost time must be reported to your workers' compensation board within the provincial deadline — often within three days.
