Field-level hazard assessment for confined-space work in Canada
A confined-space FLHA is a point-of-entry check of conditions that might have changed since the formal hazard assessment, entry plan and permit were approved. It helps the crew catch a new process line, failed ventilation, weather-driven inflow, changed rescue access or adjacent work. It is not the confined-space programme, the written space assessment, the entry permit, atmospheric test record, isolation certificate or rescue plan. If those records are required and absent, nobody enters on the strength of this form.
The Canadian Centre for Occupational Health and Safety (CCOHS) confined-space programme guidance describes site-specific assessment, roles, entry control, testing, communications and rescue. The applicable legal definition and detail vary by jurisdiction. For example, Ontario Regulation 632/05 requires a written hazard assessment and plan and separate entry permit, while WorkSafeBC Part 9 has its own requirements. Verify the current province, territory or federal rule for the actual employer and space.
Before using this FLHA
The competent person should determine whether the location is a confined space under the governing law. A tank, vault, silo, sewer, pit or similar place may contain atmospheric, engulfment, mechanical or access hazards, but the legal test is not simply “small opening”. Identify the space and the task, then ensure a current written assessment and entry procedure exist. The FLHA below only checks whether the approved controls still fit today's conditions. If not, stop and revise the underlying documents with the right specialists.

If the permit does not name the actual space, work and people, or if the rescue team and equipment are unavailable, mark HOLD. Do not invent a permit number or prefill an atmosphere result. An emergency-service phone number alone is not a task-specific rescue plan.

A changed condition is a decision point, not a tick-box exception. If a contractor begins welding nearby, a sewer inflow increases, the extraction unit stops, or a new process line is discovered, the approved permit and assessment may no longer describe the risk. Suspend entry, make current entrants safe, and obtain competent reassessment before proceeding.
Atmospheric testing record boundary
CCOHS atmospheric-testing guidance explains that a competent person uses suitable, maintained and calibrated instruments and that testing may be needed before and during entry according to the hazard. This FLHA should point to the actual test record and permit, not replace them with “air OK”. Record the tester, date/time, locations/depths, instrument identification, calibration/bump-test evidence, substances tested, result, applicable legal/plan criteria and monitoring arrangement in the dedicated entry record.

Do not insert universal “safe” percentages or alarm settings into a generic form. Those values depend on the governing law, substance, process and competent plan. The atmosphere can change after a favourable pre-entry reading.

Brief everyone on the space-specific hazards, the limits of the task, signs requiring exit, alarm response, communication failure, access control and rescue. Confirm the attendant knows who is inside and can summon the planned response. Workers and contractors should know how to raise concerns without being pressed to enter when the conditions differ from the plan.
Stop or prohibit entry if isolation cannot be confirmed; the atmosphere is unknown, outside approved limits or changing; ventilation fails; the attendant leaves; communication fails; rescue is unavailable; a worker develops symptoms; new material enters the space; or the task changes beyond the permit. Evacuate under the approved procedure, isolate the space and have the competent person reassess. Untrained coworkers must not improvise an entry rescue.
Example: tank cleaning after a process change
A crew has a permit for cleaning a drained tank. At the workface, they find a connected transfer line that was not on the isolation list. They record the line in this FLHA and hold entry. The process owner and competent confined-space person review the line and energy/material path, modify the isolation and permit, recheck the atmosphere and confirm rescue/attendant readiness. The crew is rebriefed before release. The FLHA caught a mismatch; it did not approve a new isolation method by itself.

The next shift or next entry needs a new check against current conditions and the applicable permit rules. A previous permit, gas test or FLHA does not automatically carry over. Protect completed health, exposure and medical information according to the employer's retention and privacy rules.
